Just do Schedules C and SE on the $5K–the forms’ll tell you which lines on 1040 to enter the results.
Schedule C or C-EZ for the self employment income and associated expenses, schedule SE to calculate self-employment tax for social security and medicare, and a form 1040. Your income as an employee will go on that same 1040.
